FEES The fees charged in this office are comparable to those charged by other healthcare providers in this area, with similar qualifications. Please ask to see our fee schedule. We accept cash, credit cards, and personal checks. Please note there is a $30.00 charge for checks returned due to insufficient funds.
INSURANCE COVERAGE Many insurance policies cover Acupuncture, but we do not claim that yours does. Policies can differ greatly in terms of deductible and percentage of coverage for acupuncture. We can verify coverage and submit your claim form for reimbursement, provided you sign the financial agreement below. Please provide your insurance information at least one week in advance of your first appointment. If you do not provide your information in time for our office to verify your benefits, you will be charged the time of service fee for your first appointment, and later credited if you do have insurance coverage.
RELEASE OF INFORMATION Your insurance company may require medical reports to document our treatment and progress. Your initials below authorize the release of medical information necessary to process your claim.
CANCELLATIONS As a courtesy to our office and other patients, we ask that you please notify the office at least 24 hours in advance if you need to cancel or reschedule your appointment. You will be charged a $30.00 fee for any cancellation giving less than 24 hours notice for nonemergency situations. The $30 fee will also apply to reschedules occurring within 2 hours of your appointment time unless rescheduling for the same day. A $50 no-show fee will apply for any appointment that is missed without notice.
PATIENT DISCHARGE POLICIES AND PROCEDURES
At Equinox Acupuncture Wellness we try our best to earn your trust and business, and we do our best to accommodate various reasonable requests. However, there is a time when it is not a good fit for both parties. Below are the following discharge policies for patients not complying with our clinic guidelines:
NON-PAYMENT FOR SERVICES In this case the client needs to pay the balance due before services will be resumed.
HARASSMENT AND THREATENING BEHAVIORS This includes harassment by voice, text, electronic, verbal or other means that is disruptive to staff, patients, or other tenants of 125 High Rock Avenue.
NEED FOR OUTSIDE REFERRAL If Equinox Acupuncture Wellness personnel decides that the client needs to be seen by another specialist, and possibly can be harmed by not complying with our recommendations, we reserve the right to terminate the services and refer the patient to the appropriate provider.
DISRUPTIVE BEHAVIOR Patients who consistently are late and/or let their phones ring in our office after 3 warnings of non-compliance will be asked to leave the office and treatments will be discontinued.
We do understand different needs of our clients and we ask to respect our staff, time, and policies as we respect your unique needs.
